Case Study : Aesthetics Dermal Fillers: Lips, Mid-Face, and Chin

Indications/Patient Presentation:

• 20 year old female of Spanish descent
• Desires lip augmentation as well as cheek, chin and jawline reshaping
• She conducted an extensive review of photos on Instagram and wanted a very full “French Kiss” or “Russian” lip with high cheek bones
• Budget was not a limiting factor
• She is overweight and has lost some weight recently on very low dose (2.5mg weekly) compounded tirzepatide; however, she doesn’t take it consistently due to nausea

Objective Findings:

• Physical exam is non-contributory
• Healthy female with history of obesity, she has been taking tirzepatide 2.5mg weekly SQ
• Patient has NKA and takes no other medications except birth control pills
• Patient has a thin upper lip and moderate lower lip. She has heavy fat pads in lower 1/3 of the face

Treatment:

• Topical Lidocaine/Tetracaine/Epinephrine (23%/7%/7%) applied for anesthesia (20 minutes)
• 1 mL RHA 3 to lips using French Kiss technique as described by Trevidic et al.¹
• 4 mL RHA 4 to Chin
• 2 mL RHA 4 to mid-face, placed directly on the periosteum for lift and reshaping
• Tolerated procedure well and ice gel packs applied post-treatment (see Figure 1 for before and after photos on day of treatment)

Follow-up 2 weeks later:

• Patient very happy with results
• She would like re-shaping of the posterior jawline for a “snatched” appearance after she loses some more weight

Conclusion:

• Patient happy and satisfied with appearance
